Please use one of these links to fill out your consent form before you arrive for your appointment. Use the Body Piercing Waiver form for all new body piercing appointments. The Change Out Waiver is for appointments where you will be getting jewelry inserted or removed from existing piercings.
If your appointment includes a new piercing and a change out for another piercing, you need to only fill out the Body Piercing Waiver.

Piercing Minors: Per Florida Statute, as published on Florida Health Department's Piercing Program page, "A person may not perform body piercing on a minor without the written notarized consent of the minor's parent or legal guardians, and an establishment may not perform body piercing on a minor under the age of 16 unless the minor is accompanied by a parent or legal guardian."

Pure Imagination Arts will not pierce any minor without this consent form, and without the accompaniment of the legal guardian for minors under the age of 16. Please have your consent form, already notarized, when you arrive for your piercing. If you are unable to print a copy yourself, stop by the studio and we'll happily provide with you one.
A notary can most easily be found at your local bank, or we can help direct you to someone.
